Checklist of Symptoms


Are Vision Disorders Interfering with Your Child's Ability to Learn?

___eyes strained or tired when reading ___print runs together when reading
___loss of place when reading ___reverses letters and/or numbers
___skips words when reading ___difficulty remembering words learned before
___uses finger to follow along when reading ___bright child, but does poorly on tests
___difficulty with copying from the chalkboard ___difficulty with tests using Scantron sheets
___complains of headaches after school ___avoids reading
___eyes tear or rubs eyes often when reading ___complains of blur or double vision
___holds books close to the eyes ___poor sports performance
___difficulty concentrating on homework ___diagnosed with ADD or ADHD
___distractible during homework ___diagnosed with dyslexia
___homework takes extremely long to finish ___reduced reading comprehension
___closes or covers one eye when reading

If your child experiences one or more of these symptoms, he or she should be evaluated by an optometrist specializing in vision therapy, to determine whether previously undiagnosed disorders of visual function may be interfering with your child's ability to learn.
